2.Enter your Email and Password information and click Log In.

PerfectForms Application Login Page

3.Some systems may require you to enter some additional information besides a password. This could be an e-mail address, a Staff ID or a system User ID. PerfectForms™ will then perform a lookup to validate your information. If you are unsure of the ID you need to enter, contact your PerfectForms™ administrator.

Note: It is possible to have user accounts (which use e-mail addresses as the main User ID) in more than one PerfectForms™ On-Demand account. If your e-mail address is linked to more than one account, when you log in to PerfectForms™ you will be asked which account you want to log into.
