A mobile approval form is a scaled-down version of a larger form, rendered in pure HTML without the need of an Adobe Flash plug-in. Mobile approval forms help facilitate a more efficient on-the-go approval process by allowing for quick approval or rejection of a specific request (such as a time sheet approval or vacation request). Note: To download this form, click here. Then import the form. Log on to PerfectForms™, go to the Forms Dashboard and click the Import button. Then select the form file you saved locally (.PF) and continue through the import steps. 

Enter test information into the form below and click Submit. This will send an e-mail notification to the e-mail address entered in the "Supervisor's E-mail" field. You can then access this notification using your mobile device and follow the link in the message to launch the mobile approval form.
