Does anyone have any suggestions on what to do when we take our PerfectForms server offline to do routine maintenance? Or any options in case of an unexpected shutdown of our Perfect Forms server?

in terms of unexpected problems it is best to have a good 'backup' process in place.
    see here for the basics:

the key being that all your forms/data/reports etc are all stored in SQL so having a regular backup of your database will enable you to restore back from any unexpected problems

We actually have our Perfect Forms 'web' server IIS on a separate machine. I was thinking more about instances when we would take this machine offline for maintenance, since during this period any requests for this form would return a page not found or similar error. We have a separate set of procedures in place for our SQL Server backup.
    Thanks.

if you are referring to taking the SQL Server offline, then handle this as you would for any website, perhaps setting up your customised 'not available' page presenting whatever message you want to show and copy that over the default document current used (main.html if you look at your website properties) keeping a copy of the current default document so you can replace it when you are back online again
