I would like to add a movie clip into the form.
What type of formate does it have to be in?
I have tryed .FLV but it does not seen to find the file.
I would also like to use the paypal integration. Is there a sample form I can download?

flv is supported ? and you should be able to upload these files to your form as an 'asset' and then use them within the movie object. Are you selecting the appropriate 'files of type' from the default of 'Image Files' to select 'Video Files' as you browse to find the local flv file
as to paypal integration. afraid there is no 'sample' form available but if you contact the forms design team (http://www.perfectforms.com/build-form-request.php) they could assist and build a form to your exact requirements
