Check that your mail client/server is allowing this email address ? could be that there is a ‘spam’ filter involved and it is being blocked and you aren’t being notified about this.
Consider adding @perfectforms.com to your ‘safe sender’ list.. your mail admin will be able to tell you how to do this
Then… you could go to try to log in with your email address and no password, to get to the ‘wrong password’ where you can then use the ‘recover password’ option
