   When I run the form I get "agent LDAP error" popup and then it goes to the form, but the form isn't populated.

   have a look to this post on the forum in relation to this

   key would be the log file where you will find more information to the error

   but also, before you run the form, ensure that the Directory Service connection 'CHECK' passes correctly. If you have incorrectly defined the Distinguished Name in the config.xml for instance it will fail. Your Active Directory Administrators should be able to help you to define the correct information in the config.xml
