When installing PerfectForms On Premise the installation user will require SA level access in the SQL server instance. This is required because the installer runs custom C# code in the SQL server. If the DBA does not want to grant SA access, an option in SQL server called "clr enabled" can be activated prior to running the PerfectForms installer.
