   after i log in the perfect form, and constantly receives the message states i am not log in, but i am actually on about 5 minutes.
   is it the network problem, or anything else?

   You are using PerfectForms On Premise (on your own server). If there is a network problem it is on your own network. Does anyone else have your log-in information? If someone else logs in with your credentials you will be kicked out.
