    I am not sure if this is covered in earlier post, how do you change the subject line so it does not say PerfectForms.

    You are talking about changing the “from”, not the subject line. You need to use your own SMTP server to make the emails come from your domain. There is a section in our help guide on Adding an E-mail Server as an Account Resource: /Documentation/manual/html/?fg_adding_an_email_server_account.htm
