We have been using a perfectform on a page in our intranet. The user says it was working fine with IE7, but no longer works in IE9. It does appear in Firefox. Is there an Internet Explorer setting I should look at to be able to do this in IE8?
The first place I would check is to make sure the version of Flash that IE is using is current.

My Flash was current as of August, so I went ahead and updated it and the embed is not appearing.
We don’t even get an error message.
Have the user open IE and click F12. There he can play with the Browser Modes to try different compatibility modes or versions.
None of those helped. We’re behind an ISA server proxy. But I see how that could be a problem, since Firefox gets the embed right.
I don’t have experience with ISA server proxies, but it must be a security setting in those versions of IE that is preventing the form from displaying.
